The Medicaid MFP and ABI Waiver are Person-Centered Programs. A person should never be forced, tricked, or shamed into taking action . There are two Acquired Brain Injury (ABI) Waivers, known as ABI Waiver I and ABI Waiver II, which are provided by the Federal Government and administered by the Depa rtment of Social Services (DSS). These waivers provide a person-centered plan of care for an array of non-medical, home and community-based services to individuals with an acquired brain injury who currently receive or would otherwise require services in an institutional setting. What is Person-Centered care? • Care that’s guided and informed by a person’s goals, preferences, and values. • The success of the services is measured by the person ’s reported outcomes success. • Integrated and coordinated care across health systems, providers, and care settings. • A team works together to support managing chronic and complex challenges. • Relationships are built on trust and a commitment to the person ’s long-term well-being. What does person-centered care mean for a person? Person-centered care allows a person to make informed decisions about their treatment and well-being. They have a team of primary care providers, specialists, and other health care providers who know them, listen to them, and are accountable for their care. Doctors and other healthcare professionals help person-centered care persons manage their health care by providing tools and services that align with their preferences and values so they can reach their health goals. Connecticut Conservatorship MFP provides support and resources for individuals with ABI/TBI and stroke. Our mission is to improve the quality of life for those affected by brain injury and ensure their rights are protected through effective conservatorship. Join us in our mission. ( Conserv atorship ) What is a Conse rvator? A conservator is a person appointed by the Probate Court to oversee the financial or personal affairs of an adult. In an involuntary conservatorship, a conservator is appointed only if the court determines that the individual is unable to c are for him or herself or unable to manage his or her financial affairs. In a voluntary conservatorship, the court appoints a conservator at the request of an adult who seeks assistance in managing his or her affairs without making finding that the individual is incapable. There are two kinds of conservators. A conservator of the person supervises personal affairs and ensures that the person's basic n eeds, including food, shelter, clothing, and health care, are met. A conservator of the estate supervises financial affairs, including caring for the property, managing bank accounts, and ensuring the safe handling of the person's income. Often, the Probate Court will appoint a family member of the individual, or his or her close friend, as the conservator. Sometimes the court will appoint someone else, such as a lawyer. The court tries to determine whom the conserved person prefers but, if a conflict exists, may appoint an uninterested party.
