Schedule client sessions and programming based on Individual Service Plans and training domains. Conduct in-home training, teaching, and support for clients in areas such as money management, nutrition, cooking, comparison shopping, emergency procedures, community awareness, mobility, laundry procedures, household management, sex/public health education, medical procedures, counseling, crisis intervention, and housing. Provide training in accordance with current care plans and the least restrictive environment philosophy (1:1 staff-client ratio, unless otherwise specified). Coordinate client training programs with other programs in which they are involved. Complete daily activity reports documenting direct hours spent with clients and submit to supervisor(s) on the first day of the following month. Maintain up-to-date client files. Assist clients in coordinating with other programs, services, and benefit systems, such as Social Security, AFDC, WIC, food stamps, counseling, etc. Attend monthly staff meetings and individual meetings with team members. ILST PCA Weathering a storm as a Connecticut caregiver. Lets review this checklist before the weather gets bad. Being without electricity is not an option for people who rely on oxygen machines. Closed roads and stores may mean no medications, food or supplies when there is a natural disaster. Teams that visit people in their homes are well-versed in preparing for severe weather. Extra medications and supplies may be needed before the storm hits to ensure clients are never at risk. The team may even suggest a client spend the duration of the storm in a safer location. If the forecast calls for severe weather and youтАЩre taking care of a seriously ill loved one, here are a few tips to help you weather the storm. In fact, Keep these tips in mind for any natural disaster, from fires to heat to hurricanes. Let's Be Prepared Well in advance, tell your power company if someone in the home is oxygen-dependent to receive priority service or the loan of a generator. Many communities offer programs in winter to help with snow removal and heating costs. Post the phone numbers of the clientтАЩs physician in or near your phone. Include the numbers of the local fire department, EMT, and others you may need to contact in an emergency Make sure cell phones are fully charged in the event you cannot use your landline. Charge laptops, tablets, and other devices. Ensure you have flashlights with working batteries. Have extra batteries on hand. Check that all smoke alarms have working batteries. If you use alternative heating such as kerosene, have a working carbon monoxide alarm. Keep bottled water on hand for drinking, cleaning, cooking; a gallon per person per day is recommended. Fill the bathtub with water for toilet flushing in case the power goes out. Have a battery-powered radio to stay up to date on weather conditions in your area. Fill your carтАЩs tank with gas and maintain good winter tires. If You Lose Power: Keep the client as warm as possible with extra clothing and blankets. Watch for shivering; this is the first sign the body is losing heat, which could mean hypothermia. Conserve heat by closing off all unused rooms. Place rolled-up towels in cracks at the base of doors and windows. Things to Remember If you must go outside, dress in layers; stay as dry as possible. When shoveling snow, avoid overexertion by taking frequent breaks. If you must drive, stay on main roads as much as possible.

(this means a client needs to be able to walk this distance safely). In order to deem a client eligible for a more supportive transport method a Medically Appropriate Mode Form (MAM) needs to be completed and SIGNED by a licensed professional (MD, RN, LCSW). MAM forms can take up to 15 days to be approved/denied by VEYO. If Denied, Veyo sends a letter in one business day to the client for a reason. If approved, MAM is logged in the client's file and they use the medically necessary mode of transportation going forward. These forms expire and need to be completed again after 1 year. If within the 15-day approval, time frame the client needs medical transportation, the client can ask for 1 to 15 day тАЬcourtesyтАЭ (up to 2 times in a row) to get this transportation mode while MAM is pending. If the client requires an escort/companion, a Companion Form must be completed by a licensed professional and approved by VEYO. For clients with appointments a distance away need the Closest Provider Form. This form is needed based on the following: 1) If they live in an urban area, the form is needed for appts beyond 10 miles 2) If they live in a rural area, the form is needed for appts beyond 20 miles. FOR QUALITY CONCERNS: Report Missed Pick Ups, Poor Customer Care, Long Hold Times, etc to which is their quality assurance department who will do an investigation. Include the date and time of the call as well as the agent- as the call is recorded and the investigator will pull this. Also asked to be included in the communication in the email as a representative for the client тАУ 1) Include name of driver if known, or type of transmit 2) IDPs are Independent Driving Providers (like an uber) 3) 3POs are 3rd party operators- like ACE, Metro. Clients can ask in their record for a preferred provider if they prefer someone who has provided satisfactory services. If an appointment is scheduled/confirmed and the driver is a no-show, the main number can be called and select the option for тАЬWhereтАЩs my RideтАЭ. Report the driver is a no-show and you want to file a complaint so this can be logged in the driverтАЩs file. If the ride is urgent, ask for a rescue trip to be pursued. An agent will seek a driver to arrive for pick up in less than an hour.
